IMPORTANT TIPS
1. Designated branches of Allahabad Bank will provide the INFORMATION BROCHURE after payment
of examination fee in cash, Rs.1050.00+30.00 (Bank Charges) including counselling charges (Rs. 50.00) for
General/OBC Candidates, and Rs.550.00+30.00 (Bank Charges) including counselling charges (Rs.50.00) for
SC/ST Candidates.
2. The brochure includes an application form which is in the form of an OMR sheet.
3. The Brochure should be thoroughly studied before filling the Application Form.
4. The Application Form is to be filled up on the basis of the information contained in the Brochure.
5. The candidate must ensure his/her eligibility to appear in the CPMT-2012 on his own and shall be personally
responsible for the same. The examining body shall not be responsible for this verification. His/Her candidature
shall be cancelled anytime before/after the examination if he is found not eligibile at anytime.
6. The OMR sheet is to be filled with utmost care with dark blue or black Ball Point Pen. (Do not use Felt/Sketch
pen) There should not be any overwriting or cutting.
7. The duly filled OMR Application Form, is to be submitted by the Registered/Speed Post placed in the envelop
without folding, to the Chairman, C.P.M.T.-2012, CSM Medical University, U.P., Lucknow-226 003 U.P.
8. Write your OMR Serial Number on the envelope along with your address with PIN Code and Phone number.
9. Admit Card along with your Roll Number, shall be available for download at website www.csmmu.in
10. Admit Card shall NOT be sent by Post.

Important Note regarding Photograph CPMT 2012
Photograph should be
1. High contrast 3.5×4.5cm size colour photograph with full front face facing camera,
no cap, no hat, no dark glasses and on white background (Compulsory).
2. Top of hair to bottom of chin should measure 2cm. (Compulsory).
3. Polaroid and inkjet photos are NOT acceptable.
4. Applications with poor quality photo would be rejected.
5. Photograph MUST be taken on or after 01.02.2012 and make 10 copies of the
photograph, which will be used for counselling, admission etc.
6. Photograph must be taken with a placard in front of the candidate carrying the
name of the candidate and the date on which the photograph is being taken.
7. The name and date on the photograph must be clear and legible and should not be
digitally composed after taking photo.

Application and Examination Fee
For one subject Rs. 2000/-
For two subjects Rs.2500/-
Application Form and Examination Fee shall only be submitted online. No application form shall be received by post.
Official Bank for submission of Fee is Axis Bank (any CBS branch in the country or by Master / Visa Card)
Important Information:
Online Application procedure
  • Application shall be submitted online only using Internet, this would also allow you to get a admit card online itself.
  • Online application can be submitted on website www.kgmcindia.edu / www.csmmu.edu.in
  • First register and create your login ID and PASSWORD on the website link as directed and note it down for further use.
  • Payment for examination fee can be done by Master/Visa credit card online or you will first need to deposit the examination fee in any of the Axis bank branch in India. You will be provided a receipt with Transaction ID number on its deposition. Please get scan of bank receipt for filling the application form.
  • In case payment is to be done by credit card you can proceed for filling of application form after login and payment
  • In case of payment by bank challan you need to relogin with Login ID and Password after two working days after making payment at bank and then you will be able to fill application form. Please have a scanned copy of bank receipt ready for filling the application form at this stage (Payment for the application through bank challan can only be made till : Friday, July 13th, 2012).
  • Please upload your passport size color photograph and scanned signatures on white background in the application form. The photograph should be full face, from front, with light background and without spectacles. The format (jpeg, jpg) size should between 100-200 KB, for each)
  • Fill all the entries in the application form carefully and check them before final submission as wrongly filled, incomplete application form will be rejected and no enquiry will be entertained regarding same.
  • Only for your personal reference please take a print out of the completed application form.
  • There is no need to send the printed application to the Coordinator, AIDMMChEE2012.
  • Same Login ID and Password will be required for downloading the Admit Card.
